You will need a referral from your physician or chiropractor to receive massage therapy. This referral should indicate that treatment is for rehabilitation from your injury and needs a diagnosis. Have them fax it to us or bring it with you on your first visit.
*WC Claims: You are entitled to 6 massage therapy sessions under your claim before pre-authorization is required. For further treatment, authorization will have to be approved by your Claim Manager/Adjuster.
For claims without pay-as-you-go coverage (such as bodily injury claims), or where no payment will be disbursed to providers until a settlement is reached, we limit patients to a held balance for 12 one-hour sessions unless a promissory note is provided from your attorney.

All claims are billed at $140.00 for an hour of service, as is our standard rate, and are not subject to discounts since payment is not made at the time services are rendered. We believe this to be a fair rate for the services, billing, and record handling required for claims. Services are not guaranteed to pay via your claim, in which case you will be liable for charges.
